Application & Appointment

In the United States, Guam, Puerto Rico, Virgin Islands and Canada the DAT is administered by Prometric Test Centers. Once the application is approved, the applicant will receive an e-mail or letter informing to call the Center at 800.688. A list of test centers where applicants can make their testing can be found at Prometric.
After calling, the applicant must schedule a testing appointment at one of the Prometric Test Centers; but only once the Prometric Contact Center receives authorization from the DTS. Plus, any individual local test center can schedule, reschedule, or cancel appointments.


       Applicants can submit by an electronic or paper application at using a credit card.

       Optionally, examinees may request a paper copy of the Guide and a paper application form by mail to 211 East Chicago Avenue, Suite 600, Chicago, Illinois 60611-2637, or by facsimile to 312.587.4105.

       The payment must be by money order or certified check. Only member of ADA or ASDA can pay by personal check with their membership number on the check.


The total time to complete the four tests of the DAT is 4 hours and 30 minutes. There are optional breaks of 15 minutes each, if any examinee chooses to take it.

       After processing the application and fee payment, the Prometric Contact Center will contact you by mail or letter to schedule the testing date and time; or visit
       Electronically, you will be asked to select:

         Area of study: Professional Licensure and Certification
         Region: United States
         State: state in which you choose to take the test
         Click on Next button and then click Schedule an Appointment
         Enter your eligibility number
         Enter the first four letters of your last name, if less, whole it complete

       The confirmation is made by the applicant calling the Prometric Call Center or electronically at Then confirm the details and print it.

       The cancelation or rescheduling of the appointment can be made by calling the Prometric Contact Center at 800.688.5804 or at but only within two business days prior to the scheduled testing date. Otherwise, neither the rescheduled nor the refund will be done.

       Local test center cannot schedule, reschedule, or cancel your appointment; and leaving a message is not sufficient either.


       The DAT fee is $205 and it includes:

         Submission of official score reports to five dental schools
         Personal copy of scores
         Copy of scores for the pre-dental advisor

       All requests for official score reports received after the time of application require a $25 fee per copy even if fewer than five schools were selected at the time of application.

       Fees are not refundable or transferable.

       All fees are payable only in U.S. dollars, by credit card, money order or certified check.

       All cash and personal checks will be returned.

       When credit card charge or test fee are cancelled, the applicant's score will be withheld until payment is received.

       If you have not resolved a debt within 60 days after a testing date, your scores from the test date are destroyed.
